Naturalman: 4000 BHT with Dr. Umar, 4.5 months

Tuesday, February 20. 2007
Dr. Umar mentioned:
"Naturalman underwent old fashioned plug type transplants in the 1980s which left him with egregious plugs in the entire NW areas. Several years ago, he underwent FUE, BHT, and plug removal and redistribution repair work with Dr Woods. He was quite happy with the outcome. However, he was unable to continue his repair journey because his remaining body hair was either non robust or located in the apocrine areas (armpit and pubic areas):

AT consultation, naturalman was advised to commence the use of topical minoxidil to his BHT donor areas as per our regimen. At the time of surgery, he had used minoxidil to the body areas for about 3 months. Please read the following thread for more on minoxidil and BHT:

4 ˝ half months ago, naturalman underwent the first of 2-3 repair sessions at out clinic. Part of this entailed the removal of several egregious plugs in the crown and the implantation of ~3900 fine, non robust body hair into the crown. These were derived from the forearms (>900) and the thighd/legs (~3000).

Naturalmans situation is uniques in many aspects, one of this would entail the monitoring of non robust hair BHTs as well as minoxidil enhanced body hair.

Naturalman started experiencing growth sprouts at about 2 months. Since then he has noticed progressive filling of the crown area with finer long hair.

Here are some photos reflecting interval changes at 4.5 months after surgery to the crown.

Obviously, more progress is expected in the ensuing several months

Illustrations of graft sourcing/distribution:

At this point the body hair grafts have shown more growth activity than the plug derived scalp grafts. We always caution patients that grafts derived from plugs may not exhibit optimal yield, and growth onset may indeed be delayed. The non robust forearm and leg hair grafts are growing and gaining in length. From naturalman’s experience, they are collectively making a cosmetic impact already.

Non robust graft appearance upon extraction:

Naturalman has already undergone his 2nd stage of repair work which entailed grafting of most of his scalp donor area, and rehabilitation of his frontal scalp and hairline. Updates on his latest surgery will be presented in subsequent weeks.

Several plugs were removed/debulked from the crown:

The plug extraction sites were addressed by either skin grafting (derived from plugs) or suturing:

The crown was then implanted with ~3900 grafts in a whorl like fashion. ~190 of these were derived from the plugs and were implanted to the very front of the crown, while the rest of the entire crown was implanted with non robust body hair derived from the forearms, thighs and legs.

Healing has been excellent as evidenced by the appearance at 4.5 months:

Donor healing has been excellent without further measures. “Self sealing” was manifest in the BHT extraction sites especially in the forearms.

Perhaps it should have been clarified that the ~3900 grafts were tansplanted to the entire crown, and the midscalp verging on the frontal scalp.

The coverage Naturalman is experiencing is at 4 ˝ months, and for 3900 grafts planted to the entire crown, midscalp and even aspects of the frontal scalp.

The following photographs should help:

Clarification: There has been a consistent oversight on the graft count mentioned above. Naturalman received (and our records support)~2,900 BHT grafts to the crown, midscalp and aspects of the frontal area. Over 800 were implanted to a strip scar."
