The recent discussions re: Dr. Nigam and his stem cell hair multiplication procedures have gotten hopelessly confusing.

I am sorry if this has already been answered but please don't make me go through the older threads, I have no time for that.
My questions:
1. how many patients have been treated with hair multiplication using stem cells so far? Where are the before and after pictures?
2. what is Dr. Nigam's background and credentials, how come I have never heard of his name before? How long has he been doing hair transplants?
3. what is "Hair Activation" that I read on this link, what is the difference between Hair Activation and Hair Multiplication?

No photos backing up his claim, I find it amazing
out of 250 patients who had great results not a single convincing photo

Actually, he did have some photos but every single one of them were faked. Unfortunately, once you do something like that, you lose all credibility.
On top of that, I think anyone who offers synthetic hair implants and personally undergoes a FUT procedure, while at the same time claiming to offer both hair multiplication and "hair doubling," is a charlatan.
I think the posters on here would be wise not to pay too much attention to this fraud. There's a lot of desperate hairloss sufferers out there but you can't allow yourself to be conned or put in a dangerous position.

Dear Smurf,we inject 10ml HM solution ,most of the solution, on balding and thinning areas.You don't have to worry about pain ,since we numb the scalp with EMLA cream,dermajet ,ouchless needle,vapocoolant and before we inject,nerve block can be given to make the whole scalp numb.One injection per two sqcm.At present i cannot specifically predict the hair growth in different parts and different scalps.My clinical experience and observation is ,i have noticed less growth only on temples and sometimes on vertex,all over the scalp ,hair growth is quite obvious,even with naked eye. yes u are correct,it's still few years to perfect the HM, but even if i am able to give u activated dormant telogen or new hair follicles,though number may vary,with the present success of HM,it can be used in combination of other techniques to get denser HT.And specially if u inject stem cells into normal donor and recipient of fue and donor doubling ,the results should improve even in traditional techniques.Every month we r improving,and some patients have amazing results.What u r going to experience, when histogen and aderans are going to come in the market by 2017,now u can experience a percentage of it in 2012 in MUMBAI...now..!I don't charge if u don't get hair.Take my word ,in the [color=black][/color]future like pathological labs,grafts will be sent by doc's to the lab and injected back,there will be chain of lab's competing among themselves to provide better HM process. » It sounds like this is all experimental, whatever Dr. Nigam is doing » still unproven and with no consistent results, am I correct?

JJJ,i preffered stem cell graft extraction the second time with a small fut strip,as required by the lab,i extended the strip little more so that i can fill up my vertex.if a patient wants fue only ,i will give them the same.donor doubling will be commercially available and promoted 4 months from now,what should a doctor offer to alopecia totallis, burnt scalp,lichen planus,broque and other autoimmune scalps,ofcourse no option,but biofibres,after due test patch testing for 3months to rule out any rejection.if u think i have lost credibility for u,it's better u don't visit this thread ,till i get back credibility in ur mind.[/color]thank god,for letting me know ,what i am here for ,ur so intelligent,how can anyone con u..! Dr Nigam----Could you try plucking hairs from the donor site and dipping them in the hair multipipication cell solution then implant them like a regular FUE transplant. The plucked hairs have some stem cells clinging to the follice root but not enough to grow. With the extra stem cells from the solution the plucked folicle would have a better chance to grow. The plucked hair from the donor site will grow back so there would be ulimited donor hair (except for the 200 FUE to make the hair multipication solution)

Back in 2006, Dr. Paul Kemp of Intercytex claimed to grow 66 new hairs using 100 injections, and in Aderans November presentation they describe a protocol of 10 injections per cm2 that grew 13 new hairs per cm2. Both of these pieces of information tell us HM will grow around 1 hair per injection.
If my bald spot is 40 cm2:
With Aderans protocol, I receive 400 injections for an average of 520 new hairs.
With your protocol, I receive 20 injections for an average of X new hairs?
What is X? The only hair count information we have to go on is your website which says you've seen growth of 30,000 new hairs on some patients.
How do you explain the fact that you are growing so many more hairs with so few injections per cm2?

Fantastic , MELL,actually for these feedbacks i had come to this forum,not to promote dr nigam, which some of the critics cannot understand.I will definitely try this and document and give u a feedback, theoretically it makes sense.we are already the only clinic i suppose, who is putting antibiotics like gentamicin,antimycotic into the graft storage medium,for waiting grafts in fue,because unlike most of the clinics, even if their is a minimal infection of the grafts, while they r waiting to be implanted ,results will be poor.similarly,we put arterial 250 ml PRP(not 10ml) into the graft solution even in fue ,to nourish the waiting grafts and give them the same serum environment as in the scalp,because anyway the grafts are supported by serum in the scalp till neo vascularity develops.we also put stemcell inducing growth factors ,the same that we use in our lab to improve quality of fue hg.instead of just dipping ,i prefer,activated stemcell injections on normal donor and recipient site of fue,even in non HM patients.Actually the stemcells in the plucked grafts ,as claimed by dr.gho, cannot be activated unless isolated with biological markers in a lab,that's the advantage of having a lab with the clinic. » [color=black][/color]Dr Nigam----Could you try plucking hairs from the donor site and dipping

I've shown reactions to penicillin, and I believe cefaclor even gave me a rash the last time I used it even though I used it as a child. How do I know I won't get an allergic reaction from these antibiotics?

[/color]Good question Murf,your question has an important clue to improve HM,future studies on HM will answer this. Let me explain u,at present my protocol is concentrated 10ml HM stemcell solution on the bald patch. if the bald patch is more i.e NW6,7,than i dilute my solution ,so that i have more ml to inject,but as i dilute the solution, my concentration reduces, and hence the result.Kindly let me know, aderans one injection is how much ml and approx.number of multiplied hair stemcell in one injection of aderans.it is not the nuber of injections per sqcm, but the dose or number of multiplied stemcells which r important .Since last one week i have changed my protocol,for the same reason u mentioned.To get good number of stemcells after multiplication, we r now giving HM 2nd injection after 2 months, unless the patient desires otherwise, due to shortage of time ,earlier.we don't end our treatment after injecting 2nd shot, but prefer injecting every month the inducing stemcell growth factors, extracellullar matrix and or prp.and if require repeat the HM 2to 3times in the coming months.i cannot guarantee that after x amt of dose y amt of hair will come,because i don't know, some times the scalp is full of hair with same dose, sometimes no result.

I would have to agree and writing in paragraphs would surely make it easier to read.
Dr. Nigam what is the best case scenero for a successful HM patient? What would great results compare to if you were comparing it to a number of successful grafts transplanted during a traditional FUE procedure?
There is very little documentation as far as photos go so its hard to understand how successful HM really is even with the 50% who are sucessful.
Stating that you have a 50% success rate is a bit ambiguous. What does that mean exactly when you say success? What are you comparing it against to claim a particular HM procedure as successful? Can you expound on that please?
And yes please expound on Donor Doubling if you could? Thank u

I prefer calling my Donor Doubling technique,the Hair Doubling,because my technique allows both ,donor and recipient Doubling. We perform Hair doubling in the following steps: 1)we extract a hair follicle/s by FUE from the donor. 2)The follicle is than stained with eosin and haematoxylin to identify the follicle morphometry and locate the bulge stemcells. 3)The follicle is than transversally bisected just below the bulge. 4)These two bisected parts can both be implanted at the recipient area or one at donor and the other at the recipient. 5)Few non bisected grafts are sent to the stem cell lab,for isolation and activation of stem cells to active progenitor cells,and than injected into and around the two parts of implanted follicles.Hence, either part receives the stem cells it was missing after bisection. I am in the documentation process for HAIR DOUBLING the same,should come out with my findings in next 6months,and than hopefully offer at my clinic.

With regards to the hair doubling, do you remove the complete follicle or is it a partial removal like the Gho technique?
If the complete follicle is removed, do you implant the bisected part into the hole that is left behind or is it implanted elsewhere in the donor area?
Do you implant the bisected parts on the same day or some time afterwards?
For the hair multiplication, can the process regenerate hair in the sites where follicles have previously been removed or not?

My criteria for 50% success with HM at my clinic is, after injecting multiplied stemcells, at least 50% bald area of NW1,NW2,NW3,NW4,NW5,NW6,NW7,Diffuse pattern ,Diffuse unpatterned alopecia shows avg.growth of 60+ terminal HF per sq cm. . For the thinning scalp,after injections of multiplied stemcells,there should be an increase in hair density of terminal hair by 50% per sq cm in thinning area of scalp,and increase in hair diametre by 25%.In first three months, most of the new hairgrowth is due to activation of villous or dormant hair follicles in telogen.After 6 months we start seeing new follicles in areas even without villous hair.

I have a question regarding your doubling technique. Based on what you wrote in your post, is it correct to say that for your duplication method you:
1. take a follicle from the occipital region the scalp via FUE.
2. This extracted follicle is stained with Hematoxylin/Eosin
3. The follicle is then put under a light microscope and the stem cells are identified.
4. This same follicle is then bisected and the two halves are either both put into the recipient or one half is put in the donor the other in the recipient.
5. This process 1-4 is then repeated for as many follicles as the Dr and patient agree on beforehand?
Is this a correct interpretation of your process?

Bisected follicles are implanted on the recipient area ,if the donor area is dense.If the donor area is poor,the bisected follicle with bulge stem cells and shaft is implanted in the same hole from where it was extracted from the donor area.Yes, we have seen sprouting out of hair follicle/s from the donor area sporadically after injection of activated stem cells. Can confirm the avg. percentage, after few months. We implant the bisected follicles the same day.One person extracts,the other bisects and after every hour we implant the follicles, before extracting the new ones again. I have a few follow up questions:
I looked up some information on hematoxylin and eosin staining and i'm a little confused. Can you explain how the grafts survive the H&E staining protocol? Hemalum is a complex of aluminum and oxidized hematoxylin that actually binds to nuclei and certain proteins of cells. This plus numerous of types of reagents involved in the process (acids and what not)....Surely this complex chemical treatment of the tissues renders them unviable?
How are the follicles going through all that, surviving, and then being reimplanted in the recipient area? Also, wouldn't the follicles (or bisected halves) be permanently stained?
And what about identifying the morphology of the follicle under the light microscope? I was under the impression that light microscope needs thin tissue sections? A fully intact follicle would be too thick for adequate light penetration on a microscope. Furthermore, would not the follicle be too thick to stain and/or put under a microscope?
Any information you can provide to address my concerns would be appreciated.
Sorry for all the questions, i'm a detail oriented person...especially when it comes to a procedure i'm considering. I'm just trying to get a feel for your process.

only 6 follicular units are stained if three technicians are bisecting the grafts transversally.Location of bulge region varies with different individuals.Cytokeratin 15 and CD200 are used as immunohistochemical biomarkers to identify follicular bulge where bulge stemcells reside.Following segments of the follicles are measured.1)the mean length of terminal anagen hair follicle i.e distance from the scalp epidermal surface to the dermal papilla is 4.2mm +/-0.4mm 2)the infundibulum of the follicle measures 0.8mm 3)the isthumus measures 0.9mm 4)the inferior portion is 2.5mm long. Average region of bulge after detecting with anti CK15 is 1mm below the skin surface and extending upto 1.8mm below the skin surface.On measuring the hair follicle diameter by micrometre,the hair follicle is thicker at the inferior level,this also helps us to identify bulge area of follicle.For grafts under stain we use digital camera with optical microscope.For viewing live cells we use inverted microscope and stereo microscope. This understanding of hair follicle morphometry also helps us in deciding the depth of recipient hole and the same helps us to preserve bulge stem cells while doing trichophytic closure in traditional transplants.

Ive noticed that the standard of english improves greatly when talking about scientific details. I believe this is because He or she is just re-wording another study, to give us a good impression that they have scientific knowledge.
I pointed out in another thread where Nigam copied, word for word, another study to answer a question in a technical manner. Thus giving us the impression that these people are scientists.
Pay careful attention to the post im replying too. Look at the standard of English compared to other posts which obviously her / his own words.
This is only a small part of the deception.

Why will it take 6 months to upload photos? Although in your country the internet is not your main source of business, you surely keep records of the progress of your HM patients?
If you are trying to improve, or experimenting with HM, then do you not need to document and photograph your patients? 500 is a huge number, it is very interesting that you dont have evidence.
So once again, for your own scientific purpose, do you not take photographs to see how well HM is working?

Pictures required by forum members has to be preferbally in day light, without change of hairstyle,combing in and out,in optical zoom without flash,in macro and super macro mode,10cms and 3 cms distance from scalp respectively.I can't expect my patients to click themselves with these specification and mail it to me.but definitely ,i will try to post,one photo a week. Market in my country works differently than your's.Give me time ,i will not dissapoint u. I understand your point, though i do not understand your focus on hair doubling when you have achieved such great things with hair multiplication?
I also understand it takes time to grow hair, but Dr nigam, how long have you been testing your hair multiplication?

Did you video record the injection procedure yet?
I still don't think more photos will make much difference to anyone here. The only photos I really want to see are high res photos of scars. I want to see high res photos of the scars a normal FUE leaves in the recipient area and high res photos of the scars in the donor area when you use a 1.2mm punch for your HM procedure.
Showing me pictures with no scars doesn't tell me anything because how do I know a picture of HM is not just a normal FUE if you can't show me the scars from one of your normal FUE?
If you don't want to post pictures of scars, then I think you need to show a video of the injection procedure, which is something you can do right away because you don't have to wait on old patients to come back.
I can't think of anything else that would help. Taking better before and after photos doesn't help.

Youre rright photos wont help much, given how much photos can be manipulated. But for peopel multiplying cells in a lab (no faint task) and then injecting, and then saying 50% success rate, but without taking photo evidence, strikes me as strange.

Are his stats based on people phoning up and saying "hey it worked!". Doesnt seem very scientific to me. Thats why the photo thing plays on my mind.
Id be very surprised if he would answer my query fully. He wont, such a basic question, and theres no answer.
Furthermore, if these patients get HM, go home to never return, how does this clinic ensure the treatment is safe? It seems insane to be multiplying cells and not investigating into safety (something replicel, aderans, histogen put alot of time into).
Surely they have to take samples to check for anything bad? So we have two major defects in the science of Dr Nigam...

I understand these are your two primary concerns, and I think they are good ones, but I don't think it's fair to say that no explanation has been given for all this.
The explanation is... He's not a scientist testing a commercial product. He's a doctor testing a new procedure. Those are two different perspectives.
Doctors don't necessarily collect statistical data on outcomes the way you describe, and they definitely don't go through the safety protocol your describing when they attempt a procedure that their judgement tells them is safe.
Is it safe to simply reclassify this as an new medical procedure instead of as an experimental pharmaceutical product?
Thats a good question, but lets face it. We wouldn't be talking about HM in India in 2012 in the first place if it wasn't to get around the FDA by calling it a procedure.
I'm not saying this is safe. I'm not even saying that it's not all baloney. I'm just saying it's not a completely unbelievable explanation that was given and an explanation was in fact given.

Dear Smurf,
I've dictated the script to the videographer today. Should be ready in the next 15 days. The script starts from the patient entering the reception to the patient care executive to the consultant, then escorted to the hair multiplication chamber for a pre-procedure profile, introduction to the surgeons and technicians and actual start of procedure of extraction of grafts, then transporting the grafts to the next door lab and prcessing the grafts and getting it back to the clinic and injecting onto the scalp and sending off the patient with the flower bouquet. If I get time, I will add scientific explanation on the film.As you know, some forum members are so prejudiced and always have an opinion on whatever I submit and they take it as 'deception'. But, I am loving it, the results are in front of my own eyes and I know that any new procedure will be scrutinised and doubted. Keep pushing me Smurf, it only helps me ! 1 more photo of early results and 2 videos will be uploaded on my site tomorrow. I hope to upload at least 1 good quality before/after photo every week.

I see that you moved quickly on the video front Dr. Nigam
Can I make the suggestion that for the injection procedure you use a tripod and also get up as close to the scalp as possible and let us watch enough of the injection procedure so that we can zoom in to clearly see the density of your injections and so that we can confirm roughly how long and how many shots the full procedure will take or else upload the full procedure so that we know this for sure.
Whatever you may be doing, you're at least having a dialogue in good faith so I think there is no point in trying to trick you with this video. I think It's fairly obvious now that everyone will be inspecting this video under a microscope so you might as well shoot the thing in a way that facilitates measurement in the first place just as you would do with the next round of before and after photos.

When will you re-design the website?
Your plans for the video sound great. Can i suggest that the videos should be in a 'before' and 'after' format.

Can you explain the new photo on your website of Yogish Lakhani? I dont see any improvement and the 'after' photos are very blurry. How long after HM are those photos taken?

3 months post first session of HM,the patient has to go through another 2 sessions two months apart.Early results with approx.1000 hair(dormant telogen activation)growth seen with naked eye on distal scalp and vertex,where HM injections were given. Will ask the web developer to post pics separately ,so that u can zoom and see better.Tomorrow ,i will post my pics from inside my lab working on isolating and multiplying stem cells. » Dr Nigam,
