Hair transplant has come on leaps and bounds since its early days. FUE is becoming more and more mainstream form of hair transplants. FUE stands for Folicular Unit Extraction.
The recent ISHRS Convention (that stands for International Society of Hair Restoration Surgeons convention) it was agreed that the acceptance of FUE is a lot better in the industry now and gradually the amount of surgeons performing FUE has increased worldwide also there is a bit more predictability with the outcomes in the results.
When I started performing FUE's , one of the things I learnt that not every client is suitable for FUE. There were days where the process was a cake walk and there were days where we were literally struggling the whole day. On revisiting these clients and revisiting the results and notes, one thing became conspicuous that not every client is suitable for FUE transplant and the process of filtration was introduced in the consultation process. What I noticed during the consultation process was that on average no more that 15-20 % of clients were suitable for an FUE transplant. Bearing in mind my colleagues will probably agree with this figure I gather that probably might be the industry norm in the coming years until something changes in technique and technology.
What I also understood was there were few new unexplored concepts with regards to FUE. Now lets start by visiting the donor area. The grafts in the donor area are arranged as follicular units ,in groups of 1,2,3,4,5 and up to 8 units per hair per unit. When a surgeon starts off with a journey of FUE the initial concern is to get a graft ,immaterial of the quality of the graft. I realised that with the use of few manoeuvres ,the quality of the grafts can be improved . By using 1mm punch and the manoeuvres developed by me, we were able to extract grafts containing up to 8 hair. These manoeuvres can also be applied to placement and these were the origins of the 3rd generation of FUE. When I discussed my findings with other senior colleagues in the industry they did acknowledge that the concepts were great but that would mean that the whole process would take a lot longer meaning more surgical time.
Once the client selection process has been improved the question mark the man versus supply arises here. The key thing is to counsel the patient, to give them the facts. Let's start by visiting the donor area . Following are a few concepts about the third generation FUE

FUE Hair Transplant Scars

Scar Free FUE?
Traditionally FUE has been associated with white dot scarring. There are a few factors that contribute to white dots in FUE donor site:
Size of a punch – Rule of the thumb is, bigger the punch the more chances of visible scarring.
The depth of the punch – The deeper the punch the chances of scarring increases exponentially.
Surface area used for extraction – It is important that the surgeon uses all of the permanent zone for extraction and makes sure that the punches are not placed too close to each other. If this happens there is a chance of necrosis of neighbouring follicular units which in turn leads to wastage of the donor area and that makes the scarring visible, hence defeating the purpose of scar free transplant. With the evolution of 3G FUE scar free transplants have become a reality.

Hair Transplant Ultra Dense Packing

Ultra Dense Packing
Ultra dense packing has been a controversial subject among the medical fraternity for various reasons. On the positive side, ultra dense packing can help achieve natural density in one session, limiting morbidity and saving a lot and money of time for the client.
On the negative side, there are concerns with donor depletion, limiting availability for future sessions and also the practical aspects of the procedure can be very challenging. Besides this, there is the misconception about poor uptake with ultra dense packing.
There are some issues that are addressed with the development of the 3G technique. We have known for years that more hair per graft (chunky grafts) yields better density than placing a lot of fine grafts. Let’s compare this concept to a garden. If you have a similar sized garden, you can either try to populate it with a lot of twigs, risking poor uptake or use a few twigs and plenty of trees to achieve respectable coverage.
A close inspection of the donor area reveals that there is a mixture of grafts of different sizes, namely two’s, three’s and four and five hair grafts. This mix makes the donor area look very dense and also very natural. Now if we apply a similar concept to the recipient area, there is no reason why this cannot be achieved.
However, there are a few technical issues associated with the recipient area that makes ultra dense packing a bit challenging. Firstly bald skin is exposed to sunlight and there are always issues with chronic sun exposure that makes the skin a bit harder than hair bearing skin. Secondly bleeding is more of an issue with the recipient area causing flushing away of grafts.

Afro-Carribean Hair with FUE

There are unique qualities of Afro-carribean hair that makes it extremely challenging for us to perform hair restoration on clients seeking FUE.
For starters, unlike Asian or Caucasian hair follicles, Afro-carribean hair follicles are curly and have certain unique features. On the positive side, this curliness offers great results with fewer follicles, it also poses a few challenges.
Instrumentation used in FUE is traditionally a sharp straight instrument. This makes it extremely difficult to manoeuvre the punch beneath the skin, wherein the visibility is nil. Hence historically patients of Afro-Carribean origin seeking FUE were discouraged to do so because of the high levels of transection associated with extraction of follicles using traditional FUE techniques.
As illustrated from the pictures above, using improved techniques of the 3G FUE technique, we have overcome the difficulties associated with extraction of follicles of patients of Afro-Carribean origin.
The second difficulty arises whilst placing them. The grafts are fairly chubby and hence a lot of improvisations have to made whilst replacing the grafts. I prefer to use a Choi implanter as I believe there is less trauma associated with replacing the grafts using choi implanters over using forceps.
The third issue is of achieving density. Generally I've found that achieving respectable density is a bit of an issue in Afro-carribean clients as popping seems to be an issue. I usually counsel them extensively about this and either advice them to spread the surgery over 48 hours or recommend a top up session in 12 months to achieve dense packing.
The last but not least issue is of scarring. Due to certain characteristics of Afro-carribean skin, clients have to be counselled extensively about hypertrophic and keloid scarring. I usually perform a test extraction on my patients before offering them an extensive surgery. However, with meticulous attention we have minimised scarring and have numerous examples whereby scarring with 3G FUE has been minimised compared to traditional techniques.
This client had a traditional FUE 12 months ago and Dr. Reddy performed an FUE on him recently. The scarring at the back is from the old surgery. We extracted hair mainly below the occipital notch. The picture taken a few weeks after, clearly illustrates lack of visible scarring at the back following extraction of 600 follicles using 3G FUE.
