Diabetes is fast becoming the biggest epidemic of the 21st century. Lately, the topic of doing hair transplants on people with diabetes has become a recurring subject in our forum.
Is it safe to perform hair transplant for people with diabetes? What are some of the things diabetic patients should be aware of when making a decision about hair transplant?
Hair Transplant: Type I vs Type II Diabetic Patient
Type I diabetic has insulin dependancy. These patients rely on daily insulin injection that infuses the proper amount of insulin to control blood sugar/glucose. It is harder for Type I patients to control their glucose/blood sugar. They tend to have problems with healing especially when the surface of the skin or scalp is compromised.
Type II diabetic controls their glucose/blood sugar without the need for insulin. They do that mainly with medications or lifestyle changes such as exercise or diet.
When it comes to hair transplant for diabetic patients, there is really no consensus as to what clinics should or should not do. Every clinic seems to have its own policy in dealing with hair transplants for people with diabetes.
Here we discuss how one clinic manages the pre-operative, intra-operative, and post operative protocols when performing hair transplants for diabetic patients. Hair Transplant - Diabetic Patient - Pre-operative Protocol
- Donor laxity. A diabetic patient should perform stretching exercises for a strip excision. The strip excision width should be extracted very conservatively so that the surrounding tissues will not be compromised in terms of blood circulation and to also decrease a chance of infection or necrosis. In FUE, donor laxity is less of a concern.
- A relatively good control and close monitoring by the patient of their sugar/blood glucose/insulin levels before surgery for an extended period of time.
- Manage the patient's expectation properly; advise diabetic patients about the possibility of below average hair transplant yield from the procedure.
- Check the sugar/blood glucose to see if the sugar/blood glucose is within a normal range before the surgery. Postpone the surgery if necessary.
Hair Transplant - Diabetic Patient - Intra-operative Protocol
- The blood gucose/sugar levels must be monitored continuously throughout the surgery. Insulin is adjusted accordingly if necessary for Type I patients.
- Medications that would cause a problem with diabetic patients are withheld. Steroids such as prednisone, etc are not prescribed.
Hair Transplant - Diabetic Patient - Post-operative Protocol
- Advise the patient to continue to monitor his blood glucose/sugar closely and make the necessary insulin adjustments if necessary.
- Advise patient to monitor signs of impaired healing, infection, fever, etc… And if anything seems abnormal then they must call the hair transplant doctor or his primary care physician immediately or even check in to the hospital for immediate care.
Safe Hair Transplant for People with Diabetes?
Hair transplant is a surgical procedure. There is no such thing as a safe transplant for diabetic patients. All surgical procedure carries risks. Hair transplant is not a medical necessity. When in doubt, it is best not to proceed with the hair transplant procedure. If you insist on getting a hair transplant procedure, consider a procedure with as few grafts as possible to minimize the possibility of complications. Can People Die from a Hair Transplant?
One may wonder, can someone die from a hair transplant? Apparently yes.
Hair transplant is the process of transplanting donor follicles from one part of the patient's scalp or body to balding areas in his scalp. When done correctly under the hands of a skilled surgeon, hair transplant is by far the most effective treatment for hair loss in men. The treatment can work very well for women hair loss as well in many instances.
While hair transplant is a surgical procedure in itself and as such, it does come with risks of complications, death as a result of hair transplant is indeed extremely rare.
Hair Transplant Death - Lost His Hair, Then His Life
Tragic hair transplant nightmare; story broke out in our forum about a man who dies after a hair transplant surgery at a private clinic in India.
The patient was a 43 year old man who went for a hair transplant surgery in Mar 2019. He died 12 hours after the hair transplant procedure.
Hair Transplant Death at Dr. Vikas Halwai's Clinic, India
It is reported that the hair transplant procedure was performed at Dr. Vikas Halwai's Skin & Hair Clinic in Mumbai, India. The patient paid 500,000 rupees or approx US$7,250 for the cosmetic procedure. Tragically, the man did not survive to see his new hair.
Cause of Hair Transplant Death
The investigators are still examining the cause of death at the time when this is published.
Some suggest that because the patient went for a megasession of 9,000 grafts in one sitting for his procedure, he was at a higher risk for complications. A typical hair transplant procedure averages about 3,000 grafts in one sitting that lasts about 4-6 hours. Anything above 6000 grafts is considered a megasession.
Experienced doctors have no problem performing megasession for their patients on a routine basis, but as a general rule, most doctors tend to stick to the average session of about 3000 grafts or fewer.
However, it is unlikely that the megasession procedure itself is the direct cause of death for this patient because complications were reported at approximately 3,700 grafts into the 9,000 grafts procedure.
Many sources suggest that a more probably cause of death was the result of a severe allergic reaction that that the patient developed in response to the anesthesia. It is possible that excessive amount of anesthesia that was used in preparation for a 9000 grafts megasession might have played a role in his demise, an incident of overdosing in anesthesia.
The patient reported symptoms of neck pain, trouble breathing as well as swelling on the face and throat; doctors speculated that a severe allergic reaction known as an anaphylactic shock is the most likely cause of death. Anaphylaxis is a severe, potentially life-threatening allergic reaction. Complications can occur during or after the hair transplant surgery.
Other speculative allegations include the cleanliness of the clinic, ie: whether the clinic and the equipment were properly sterilized. The possibility of strokes or heart attacks cannot be ruled out either.
Another speculation was whether adequate pre-op screening was done prior to the surgery to determine if the patient was indeed a good candidate for hair transplant procedure. Most hair transplant clinics would discuss a patient's underlying medical conditions in sufficient details prior to booking an appointment for surgery. In addition, the patient is asked to submit blood work for evaluation as part of the pre-screening process. It is unclear whether these protocols are being followed at Dr. Halwai's clinic prior to surgery.

Hair transplant is a surgical procedure and it is inevitable that the patient will be left with certain scars whether big or small, visisble or not. This will happen even under the hands of the most skillful doctors in the world. Any doctor or clinic who advertises that they offer a 100% totally scar free procedure is simply not telling you the truth.
Hair transplant scars can happen both in the recipient site and the donor site. Scars in the recipient site can be in the forms of plugs, ridging, pitting. On the other hand, hair transplant scars in the donor site are mostly in the form of donor strip scars from FUT or strip harvesting procedures. Hair transplant donor scars can also be in the form of visible white dots due to poorly executed FUE techniques.
How To Fix Hair Transplant Scars - Donor Site
Hair transplant donor scar is by far the most sought after repair procedures from hair transplant patients worldwide. In this article we focus our discussion on fixing hair transplant scars in the donor site as a result of strip or FUT procedures. This is commonly referred to as strip scars or linear scars in our community. This type of scars typically go across the back of the patient's head from ear to ear and can widen over time. We will discuss 4 ways to repair, fix or camouflage hair transplant donor strip scars.
1. Grow Your Hair Long
The easiest and least expensive way to fix a hair transplant strip scar is to simply grow your hair long to camouflage the problem areas. This is probably what most people do until the scar has widened over time and make it impossible to hide anymore. Also, some may just get tired of having to constantly remind their barbers to keep the hair in the back long in order to hide the scar, and that's when they decide to do look for a more permanent fix.
2. Camouflage with Hair Fibers or Spray
If your barber happened to cut your hair too short in the back and caused the strip scar to show, a fast and easy fix would be instant hair fibers or spray paint for thinning hair. This is an easy and very inexpensive fix. There are plenty of hair thickening fibers on the market and you simply sprinkle the tiny microscopic powder like hair fibers to the problem area and your problem will be solved almost instantly. Hair thickening fibers come in many colors nowadays so matching that to your natural hair color is never a problem. However, please note that this is a temporary fix only. The hair fibers will come off when in contact with water or soap so you almost need to reapply daily.
3. Hair Transplant Scar Excision
Now we get into something a bit more invasive if you are looking for something other than a temporary fix. One option is hair transplant scar excision, ie: removing the existing linear scar by means of another procedure in the donor site, almost the same as how the strip hair transplant was originally performed except that this time the doctor just moves the scar tissue. Healing time can be an issue as the patient will need to wait about a month for the donor site to heal after scar excision. The procedure can be costly as it is another surgical procedure that is carried out at the doctor's clinic. Also, this method does not completely remove the strip scar. It simply removes the excess scar tissue to make the existing scar look smaller or narrower in width across the patient's head. The goal is to reduce the existing linear scar to a pencil thin scar so that it is easier to hide. The patient is still left with the closure of the excision which over time may widen and become a problem again for the patient.
4. Scalp Micropigmentation SMP
Scalp micropigmentation or SMP is a recent technology in hair restoration that involves the use of tattoo to simulate hair follicles or hair growth on a person's scalp. When done properly, the result can be very natural and almost undetectable to the eye. The ink can be permanent or semi permanent. The procedure is relatively invasive and the result can last a long time. Below is a hair transplant strip scar repair that was performed using The Weston System in scalp micropigmentation.

5. Strip Scar Repair using FUE or BHT
The last and final option is to plant FUE grafts into the strip scars. This is probably the most effective way in fixing hair transplant strip scar problem permanently. This option will also produce the most natural results when done correctly. By planting fresh grafts into the scar area, this can break up the scar tissue and regenerate hair growth in the problem areas. For patients with insufficient head donor, doctors can transplant beard or chest donor grafts into the scar tissue. Beard grafts, in particular, are known to be particularly robust for repair sessions of this nature. One potential problem with this treatment is that if we are dealing with keloid scars or thick scar tissues that are completely void of blood circulation, then it may take many more donor grafts in order to generate the yield or regrowth necessary to break up the scar tissue.

Do you get a Haircut before a Hair Transplant?
If you have scheduled an upcoming hair transplant procedure, you need to know how to wear your hair before going into the hair clinic on the day of the procedure. This involves the big questions of how long you should grow your hair before the procedure and whether you should get a haircut just before the hair transplant.
The answer: it depends.
How Bald Are You Before the Hair Transplant
If you have extensive baldness prior to the hair transplant procedure, for example, a Norwood 5 or 6, then a haircut or not is irrelevant. It does not matter how long you wear your hair going into the procedure. There is not enough hair on your scalp to make any difference either way.
However, say if you are a Norwood class 3, you still have a fair amount of hair going into the surery. Then it will make a difference whether you have long or short hair going into the procedure.
Long Hair vs Short Hair Before Hair Transplant
From the hair transplant doctor's perspective, it is best that you have relatively short hair prior to the procedure. The reason is that shorter hair will make it easier for the doctor to extract the donor grafts and transplant them to the recipient sites. Longer hair will get in the way especially with the extraction or donor harvesting process.
From the patient's perspective, longer hair prior to the procedure will function as a great camouflage post op. If a patient is able to keep his hair long, it will cover the donor site nicely without being detected by friends or families. The patient can resume his daily routine faster.
Haircut After a Hair Transplant
How long do you need to wait to get a haircut after a hair transplant? The longer the better. You definitely do not want to go for a haircut just one week after a hair transplant. Post op hair transplant patients are typically left with scabs all over the donor site and recipient site. Besides, the freshly transplanted grafts need time to take and you definitely do not want to do anything that may dislodge the transplanted grafts or damage the area.
Another concern is possible infection caused by shears or cutting tools that are not properly sanitized at the barber shop. Last thing you want is to spend thousands for a hair transplant procedure and have it ruined by a haircut soon afterwards.
As a general guide, it is best to wait until all the scabs on your head have fallen off before getting a haircut post op. This usually takes about 3 weeks after your hair transplant procedure.
